The commitment of the Health and Wellness

committee is to provide holistic health education information and outreach to all members of the South Zumbro community. We commit to the development and continuity of the Parish Nurse Program, guiding the Parish Nurse in decision-making.

Access

 

A referral is received from the Pastor or parishioners. Contact the church office at 507-365-8110 to make an appointment. The next step is to call the referral participant or make an initial visit. Follow-up calls are made to meet once again where assessment forms are filled out and brochures dropped off. The number of visits depends on the level of need presented. The amount of care/visits is based upon the participant’s perception unless a family member intervenes.
